Position Summary

Key Responsibilities

• Conduct personalized Wellness Coaching sessions for individuals and corporate clients.
• Support Chronic Disease Management programs including hypertension, diabetes, and lifestyle-related conditions.
• Develop individualized Health Improvement Plans based on wellness assessments and screening outcomes.
• Monitor client progress and provide ongoing wellness support.
• Coordinate referrals and follow-up care with healthcare professionals.
• Deliver Health Education workshops, wellness presentations, and corporate health talks.
• Conduct Psychological Assessments and provide basic counselling services.
• Design and implement Mental Health Programs focused on stress management, resilience, and burnout prevention.
• Promote preventive healthcare and healthy lifestyle practices.
• Maintain accurate client records and wellness reports.
• Participate in community health awareness campaigns.
• Support organizational wellness initiatives and employee wellbeing programs.
• Analyze wellness outcomes and recommend intervention strategies.

Educational Requirements
